The delivery - there are TWO options for this:

- Pay P+P which includes all of the printing and lamination and postage. On payment clearing, I will post the form to you within TWO working days (longer if you choose to have a Logo included, whether your own or designed). Laminated documents will put you in good stead; protection for many years. Just photocopy.... and you're on your way!
